Care has become difficult and I know we need help. How do I access support services for myself or a family member?
Just give us a call and we’re happy to conduct an assessment for you or your loved one to determine if you qualify for LifeCircles PACE. We are happy to conduct tours of our state-of-the-art facility and explain how we have created a different and better kind of care for the elderly. Through LifeCircles PACE you will experience a program that provides and coordinates all of the care that your loved one needs, keeping him or her at home instead of in a nursing home. We provide:
- Primary medical care and specialty care as needed
- Routine preventative care such as dentistry, optometry, and podiatry
- Rehabilitation therapy (physical, occupational, speech) and recreational therapy
- Home health care and personal care
- Social services
- Nutrition services and take-home meals as necessary
- Transportation to and from the center; to and from scheduled off-site medical visits; and emergency medical transport
- Acute hospital and nursing home care
- Prescription drugs and necessary health care related equipment and supplies
What if my elderly loved one is currently living in a nursing home or long-term care facility? Is the LifeCircles PACE program an option?
If your loved one is currently living in a nursing home or long-term care facility but will be returning to a community-based living arrangement (such as your home or their own home) he or she can be referred to LifeCircles PACE. Just give us a call or send us an e-mail for a self-referral or talk with the social worker at their current facility. LifeCircles PACE services do not begin until the person is living in the community, but we are happy to work with the care facility social worker to begin planning an appropriate transition for your loved one. In this manner, we can assure that the progress made by your loved one while residing in the care facility is continued once he or she is home or living within the community.
How much does the LifeCircles PACE program cost?
If you or your loved one qualifies for the LifeCircles Program of All-inclusive Care for the Elderly, then ALL costs associated with the program are covered at 100% if Medicare and Medicaid eligible. Once enrolled in LifeCircles PACE, your medical, dental, and prescription needs are included in the program, as long as your care is accessed through our coordinated network of providers. There could be fees withheld from your loved one’s income when nursing home care is required.

What does “certified by the state to require nursing home care” mean? How can I get myself or my loved one certified?
You do not have to do anything personally to get certified by the state for nursing home care other than meet with our intake coordinator and answer a few questions. We complete a form and submit it to the state on your behalf. The PACE model of care maximizes available resources for health and long term care by keeping seniors at home and out of nursing facilities.
We need help with transportation for my loved one to the LifeCircles PACE facility. What options do we have and how much does it cost?
LifeCircles PACE is happy to coordinate with you to provide transportation to our facility. We work with Pioneer Resources in Muskegon to assure that every participant has a safe, handicap accessible, and reliable ride to our facility as well as to all medical appointments outside of the LifeCircles PACE center. An added benefit is that LifeCircles PACE not only provides transportation to outside appointments, but staff attend the appointments with participants and family members if they are available.
What is involved in the enrollment process?
Potential participants in LifeCircles PACE must be:
Interested individuals or their loved ones should contact LifeCircles PACE and speak to the Intake Worker regarding these requirements. If the criteria are satisfied, a home visit will be scheduled with a LifeCircles PACE nurse and social worker to answer questions and further verify the individual’s eligibility to participate in the program.
- 55 years of age or older
- Certified by the State of Michigan to require nursing home care
- Able to live safely in the community
- Currently living in our West Michigan service area, specifically Muskegon County and the following zip codes in Northern Ottawa County (49403, 49404, 49409, 49417, 49448, 49456)
Once enrolled, how long can my loved one remain in LifeCircles PACE?
Participants may remain in the LifeCircles Program of All-inclusive Care for the Elderly for life, as long as they continue to meet PACE program eligibility guidelines. Once we’ve established eligibility, it is our commitment to work with participants and their families to maintain and improve their health status. And, when necessary, to support the family with end-of-life care choices.
What does “all-inclusive” mean?
“All-inclusive” care is the cornerstone of the LifeCircles PACE model. PACE brings together a coordinated team of doctors, nurses, social workers, and therapists to provide frail seniors with an individualized care plan that addresses each person’s unique medical care and social needs.

Will I continue to see my own physicians once I join the program?
No, once you’ve joined LifeCircles PACE you will be under the care of our program. LifeCircles coordinates all of your PACE/medical care and refers you to specialists within our network as needed. All of your primary care medical appointments will be conducted at the LifeCircles PACE facility. If you choose to obtain medical services outside of our network you will be liable for any costs associated with that visit, except in an emergency.
